If you get this error when you attempt to open one of our forms, do the steps below. NOTE: Adobe Reader® must be installed on your computer.

 

  1. Hover anywhere above the page.

  2. The page header will appear.

  3. Click the Download icon.

  4. Save the file.

  5. Check the browser or file folder to see if the file has been fully downloaded. If there are no error messages, the file is fully downloaded.
  6. Go to Downloads folder to open with system viewer.
  7. The form should now load in Adobe Reader®.
